What Is a High BMI Tummy Tuck?

A high BMI tummy tuck is a variation of a standard tummy tuck (abdominoplasty) designed for patients with a body mass index over 30. This surgery removes excess skin and fat from the abdomen to make it look slim and fit. It also tightens abdominal muscles that may have weakened due to pregnancy or weight changes. A high BMI tummy tuck addresses cosmetic issues that cannot be fixed with weight loss alone.

Preparation for High BMI Tummy Tuck

Preparation for high BMI tummy tuck begins with an initial consultation to discuss the patient's goals, evaluate their health, and determine if they are a good candidate for surgery.
Dr. Askari examines the patient and the extent of excess skin and fat in the abdominal area to assess the complexity of the procedure. Key aspects covered during the consultation are: the expected contour and results; where the incision locations will be; the recovery process, including expected downtime.
If the patient decides to proceed, standard preparation involves getting medical clearance from their primary doctor, stopping smoking for 4 weeks, and following our instructions on medication use and other issues.

High BMI Tummy Tuck Procedure

The high BMI tummy tucks are performed under general anesthesia to keep the patient comfortable and pain-free. Our anesthesiologist uses appropriate medication to put the patient into a deep sleep before the surgical procedure begins. Vital signs, such as the heart rate, blood pressure, and oxygen levels, are closely monitored throughout the procedure.
Dr. Askari creates a long incision just above the pubic area, stretching from hip to hip. A second cut is made around the belly button to reposition it. These incisions allow the plastic surgeon to remove excess skin and perform other crucial procedures.
The abdominal muscles (rectus abdominis) are tightened by stitching together the fascia. This helps create a flatter, firmer, and more toned appearance. Any separated muscles are stitched back together.
Dr. Askari detaches loose skin and fat from the abdominal wall through the long incision. Liposuction may also be used to remove deeper fat deposits. The remaining lower skin flap is then stretched taut, and excess tissue is surgically cut away.
The belly button is repositioned with a small incision in the appropriate location on the abdominal flap. It is then stitched into its new place.
Finally, Dr. Askari carefully closes the incisions with sutures, surgical tape, and skin adhesives. Temporary drainage tubes may also be inserted.

Recovery After High BMI Tummy Tuck

In the first week after surgery, patients can experience some pain and discomfort. The abdomen will be swollen and bruised. Patients are advised to rest in bed or move around slowly in a slightly bent-over position, as too much activity can affect internal stitches. Medications are used to manage pain. There may also be drains that prevent excess fluid buildup. Walking should be kept to a minimum with assistance. Activities are restricted besides getting out of bed for basic needs.
During the second week, swelling and bruising start to decrease. Pain medications can be reduced as discomfort gradually subsides. Low-impact activities like short, slow walks are encouraged to promote healing. Any straining or lifting should still be avoided. Some tightness, numbness, or burning sensations may start to be felt as nerves regenerate. Patients can usually return to desk work or other light stationary activities after 10-14 days of recovery.
Most people experience a considerable improvement in mobility in the third week after plastic surgery. The swelling continues to go down, and bruises fade. Moderate movement and walking are permitted, but anything too strenuous could impact healing. Some pain or tightness is still common when bending over or standing up straight. Patients may notice increased sensation and tingling around the incisions as nerves reconnect.
Residual swelling continues to resolve during the fourth week and beyond. Patients regain strength and energy to return to normal routines with Dr. Askari's clearance. Any lingering pain, numbness, or strange sensations typically diminish over the next several weeks. Final results emerge after 3-6 months as the swelling dissipates and the abdomen fully adapts to its new shape. It is best if patients visit all follow-up appointments to monitor progress in their recovery.

Realistic
Expectations

Patients should have realistic expectations about the surgery and recovery process. They must understand that the procedure entails risks given their high BMI and that they may need additional cosmetic procedures to fully achieve their goals over time. A high BMI tummy tuck is a contouring surgery - it should not be perceived as a method of losing weight.
